So when you paying Medicare premiums in 2025 IRMAA would consider your income in 2023 With a 2 year look back, its not uncommon for the income for the look back year to be quite higher then the year you're paying your premium That's why you're allowed to appeal your IRMAA determination when the change is due to certain events

Since you have a qualifying event for the 2026 Medicare year, your 2026 Medicare premiums will be determined only by your 2026 income estimate as a single that you provide as part of the appeal of the IRMAA that was based on your 2024 income
