If you need to file a complaint about a motor vehicle transac on, please complete Form TC‐451, Original Complaint Report and submit it by postal mail to the address on the form, or by email to mved@utah gov

This number must be written on the temporary permit and the acronym USTC (Utah State Tax Commission) listed in place of the dealer name The permit stub must be mailed in to MVED DO NOT TURN THE EXTENSION INTO THE DMV WHEN LICENSING There is not a fee for an extension
